Independent practices participating in the expansion receive access to operational support, data-sharing protocols, and specialized referral channels. The non-profit structure allows participating physicians to maintain full clinical ownership and operational independence without financial oversight from private equity firms or corporate health system shareholders.
Transitioning a patient from a post-acute facility back into their home environment requires structured communication between all treating clinicians. Standardizing care plans across independent medical groups and post-acute operators allows local care teams to execute consistent treatment strategies while keeping practice decisions in the hands of attending physicians.
